I install products sold on our website and when sold we would like the customer to choose 2 installation dates a primary and a secondary date there is no option for delivery dates on the customer shipping form. Is there a work around where I would be able to add these fields to the shipping form?

simply add it as a "feature" option with customer text. Feature 1- 1st Choice installation date, feature 2 - second choice installation date
